For Chronic Care Managment using Pharmacogenomics:
CCM is a service that is covered by Medicare & other private insurances.
Essentially, it is a program where patients are offered the service at a doctor’s visit. It has to be an in-person visit to offer that to the patient. The patient consents, it does not have to be a written consent, but they verbally consent.
And they have to have at least two chronic conditions.
Pharmacist will take over this CALL pts once a month. go over their medication / reconcile their medication.
This is especially important for pt that go through transition of care. Like they go from inpatient rehab to ALF or home.
